The project involves the conception, prototyping, testing and validation of an advanced and complex technological platform of services intended as a tool that enables the management of chronic conditions with specific reference to chronic pathologies that have strong social impacts: cardiovascular, neurological and pneumological.
The project aims to support patients and healthcare workers with advanced IT tools and services aimed at improving the harmonization and sharing of processes and activities involving healthcare and social care facilities across the territory, and to monitor the adherence and effectiveness of therapeutic treatments carried out in the de-hospitalization phase in accordance with the Diagnostic Therapeutic Assistance Pathway/Individual Assistance Pathway (PDTA/PAI).

The use cases will involve patients suffering from identified chronic disabling pathologies with a strong social impact – cardiovascular, neurological and pneumological diseases – and will be allocated in the target regions (Calabria, Campania, Apulia and Sicily) and therefore involving more multidisciplinary and multi-professional integrated health networks with clinics, general practitioners, specialists, social-welfare cooperatives, etc. that are dedicated to taking care of and managing PDTA shared between hospitals, clinics and territories within the same Network.
In this project, BV TECH will deal with the collection and analysis of functional requirements, with the development of the PDTA model for the management of chronic pneumology. It will also take care of the design, prototyping and testing of the integration of transactional data of clinical systems; of the historicization and multidimensional consultation of clinical health data and of the design and creation of mobile Apps for the personalized management of therapies.
